Job Summary:

The Titration Clinician is a key member of the ADHD360 clinical team, responsible for supporting patients through the medication titration process following an ADHD diagnosis. The role focuses on ensuring a safe, effective, and patient-centred journey toward stabilisation and optimisation of treatment, in line with ADHD360 policies and formulary guidelines.

Essential Duties and Responsibilities:

  • Manage a caseload of patients diagnosed with ADHD, supporting them throughout the titration process.
  • Assess patient progress and make appropriate, evidence-based adjustments to treatment plans.
  • Conduct regular follow-up consultations (typically every three weeks), including virtual appointments and self assessed follow-ups (SAFUs).
  • Ensure all clinical decisions, treatment plans, and patient interactions are accurately documented within Chrysalis.
  • Prescribe medication in line with legal, regulatory, and clinical standards.
  • Ensure prescriptions are issued accurately and within agreed timeframes to avoid delays in patient care.
  • Ensure prescriptions are logged and recorded in line with company processes, including the postage and handling.
  • Review patient-submitted observations via the patient portal, ensuring values fall within safe and acceptable parameters for both adults and children.
  • Access and review Summary Care Records to support safe prescribing, particularly for controlled medications.
  • Collaborate with internal teams, including Quality Assurance, Secretarial, and Prescribing teams, to ensure accuracy and efficiency in prescription processing.
  • Act as a point of contact to resolve clinical or administrative queries and support patient-specific requests.
  • Promote clear, timely communication across departments to maintain a smooth and consistent patient journey.

Qualifications and Experience:

  • Current registration with a relevant professional body (e.g., NMC, GPhC, or equivalent).
  • V300 Independent Prescribing qualification (or equivalent).
  • Demonstrated experience in clinical prescribing, within mental health, ADHD, or a related field.
  • Strong knowledge of clinical governance, legal prescribing requirements, and safeguarding practices.
  • Proven ability to work effectively within multidisciplinary teams to deliver high-quality, patient-centred care.
  • Confidence in using digital clinical systems and patient management platforms.
  • Experience in ADHD services.
  • Previous experience working with pharmacies or prescription processing workflows.
  • Familiarity with ADHD 360 systems (e.g., Chrysalis) or similar platforms.
  • Private Controlled Drug PIN.

  • Attention to Detail: Ability to accurately identify and resolve errors in prescriptions or patient information while ensuring thorough documentation.
  • Communication Skills: Strong verbal and written communication skills to liaise effectively between multidisciplinary teams, pharmacists, and prescribers.
  • Problem-Solving: Proactively identifies issues (e.g., rejected prescriptions, system bugs, or delivery errors) and implements timely, effective solutions.
  • Patient-Focused: Commitment to improving patient satisfaction by facilitating smooth and efficient prescription processing.
  • Team Collaboration: Works effectively across teams, maintaining clear communication and coordination between ADHD 360 and Chemist4U.
  • Organisational Skills: Manages workload efficiently, ensuring tasks such as daily monitoring, re-writes, and query resolution are completed promptly.
  • Adaptability: Demonstrates flexibility in handling urgent requests, safeguarding concerns, or system/process changes.
  • Professionalism: Upholds high standards of care, confidentiality, and integrity in all patient-related activities.
