Job Summary:

The Patient Safety Response Lead plays a key role in the operational delivery of patient safety and safeguarding across the Health Division. The role provides expert, real-time support to clinicians and teams to manage risk, safeguarding concerns, and complex cases safely and effectively.

Acting as a central point of expertise, the postholder supports clinical decision-making, coordinates responses to risk, and ensures timely and appropriate escalation.

Working closely with the Head of Patient Safety and Safeguarding, the role contributes to the delivery and continuous improvement of patient safety processes in line with key national patient safety and safeguarding strategies, including the Patient Safety Incident Response Framework (PSIRF), supporting organisational learning and the embedding of safe practice.

Essential Duties and Responsibilities:

  • Provide expert advice on complex patient safety, safeguarding, and mental health risk issues
  • Lead coordination and escalation of high-risk or urgent cases, including direct patient engagement where needed
  • Liaise with external partners (e.g. GPs, crisis services, local authorities) and represent the organisation at safeguarding meetings
  • Maintain oversight of waiting list risks, ensuring appropriate identification and management of higher-risk patients
  • Oversee and support Clinical Referral Coordinators, ensuring consistent triage and escalation processes
  • Monitor emerging risks and provide real-time guidance to staff, supporting development through training and case discussions
  • Collaborate with governance and operational leads to identify and address patient safety concerns
  • Lead or support patient safety incident responses and investigations in line with PSIRF
  • Analyse incidents and safeguarding activity to identify themes and improvement opportunities
  • Deliver audits and ensure learning is embedded into practice with measurable improvements
  • Contribute to the development and maintenance of patient safety and safeguarding policies

Qualifications and Experience:

  • Professional qualification in a relevant field (e.g. Mental Health Nurse, Social Worker, or equivalent) with registration
  • Significant experience in safeguarding (adults and/or children) and/or mental health risk management, including supporting complex cases
  • Excellent communication and decision-making skills
  • Demonstrated ability to make autonomous decisions in complex and high-risk situations
  • Ability to work independently and manage competing priorities
  • Experience delivering training or supporting staff development (Desirable)
  • Familiarity with national patient safety frameworks (e.g. PSIRF) (Desirable)
  • Experience working within neurodevelopmental services (e.g. ADHD) (Desirable)

This role works across the Health Division and supports the delivery of safe, effective patient safety and safeguarding processes across the organisation.

The postholder provides operational leadership and support, working under the direction of the Head of Patient Safety and Safeguarding, who retains responsibility for strategic oversight, governance, and framework development.

This role may involve occasional travel for meetings, training, or external agency engagement.

Due to the nature of the role, the postholder must always maintain strict confidentiality and professional boundaries.
